Body Type, Personal Style & Color Season Intake Form
Instructions For Body Type Photo:
1. Please follow exact instructions below:
Pull your hair back.
Please wear tighter fitting clothing that is strapless or very thin sleeveless straps (a two piece bathing suit or bra/underwear is ideal) so I can clearly see the lines of your body including hips, shoulder shape, waist, hips, etc.
No selfies as this can warp the lines of your body, use the front facing camera.
Place the camera at chest level at 90 degrees to ground - make sure phone is completely straight up and down and not tilted at all. You can use a self timer or have someone else take it.
Stand facing straight on to the camera with about one foot width width apart.
Have your arms resting at side but make sure they do not cover any part of the body.
Make sure to get a full length shot - do not cut off head or feet!
Instructions For Color Season photos needed:
1. A photo of your face with hair pulled back and no make up on in natural lighting:
Please wear a white shirt and place a white piece of paper under your face.
Please ideally take this photo outside when it is light out. Avoid super direct sunlight, under shade facing light is the best option.
You may also take it inside and stand about 1-2 feet away from a window when it is light out. Make sure to turn off all overhead or artificial lights.
Please make sure there are no shadows on your face.
Make sure to not have any makeup on!
2. A close up photo of your eye pattern as I look at the PATTERN on your eyes to help determine your Color Season.
Please take this photo outside avoid super direct sunlight, or facing a window 2-3 feet away
Important - DO NOT USE SELFIE MODE for this photo as it will turn out too blurry!
See below for easy instructions on how to take this photo.
HOW TO TAKE THE EYE PATTERN PHOTO
If you are taking it yourself:
Open your camera app
Hold the phone up to your eye and make sure the camera is FACING YOU. Make sure to use FRONT FACING camera, not selfie mode!
You can use a mirror to set up shot to make sure eye is in the frame. Do not zoom in to try to get closer, this will blur the image
Keep the camera as close to your face as possible.
Then keep camera in place and move outside or close to a window to take the photo in natural light.
If someone else is taking it for you:
Have them hold the camera up to your eye as close as possible to your eye and take the photo.
Do not zoom in to try to get closer, this will blur the image
